- Road Test Appointment Confirmation Sheet
- Valid Texas Driver Permit
- $11.00 DPS Fee
- Road Test Service Fee Payment
Foreign Adults Non U.S. Citizens Must Bring the Following Proof:
- Road Test Appointment Confirmation Sheet
- Valid Texas Driver Permit
- Valid Work Visa, Student Visa, Pass Port or Green Card (which ever applies to you)
- $11.00 DPS Fee
- Road Test Service Fee Payment
Teens Age 16 or 17 Must Complete All Texas Drivers Ed Requirements and Must Bring the Following Proof:
- Road Test Appointment Confirmation Sheet
- Notarized DPS form DL-40 (form can be obtained from DPS Office)
- Current VOE Form Signed By Your High School
- Valid Texas Driver Permit
- $11.00 DPS Fee
- DE-964 Classroom/In-Car Training Certificate
- Completed and Signed 20 Hour Log Notarized
- Smart Choice Driver Road Test Service Fee Payment
Parent Taught Teens Must Bring the Following Proof:
- Road Test Appointment Confirmation Sheet
- Notarized DPS form DL-40 (form can be obtained from DPS Office)
- Valid Texas Driver Permit
- Current VOE Form Signed by Your High School
- $11.00 DPS Administrative Fee
- 32 Hour Classroom Log Form DL-90a and Log Form Affidavit DL-91a Notarized
- 14 Hour In-Car Training Log Form DL-90b and Log Form Affidavit DL-91b Notarized
- Completed and Signed 20 Hour Log Notarized
- Smart Choice Driver Road Test Service Fee Payment

Qualifications of a Safe Beginning Driver require each student to:
- Complete at least one qualifying lesson behind the wheel with a SmartChoice Driver School Instructor
- Maintain lane positions and a safe speed consistently
- Accelerate and brake with control
- Control of steering turning corners
- Complete proper and safe lane changes
- Knowing when it is safe to go
